It’s annual benefit enrollment time for health, dental and flexible benefits!

Check List

Annual Enrollment Information: This is an annual enrollment period for health and dental insurance.  If you are not interested in making any changes to your health/dental insurance at this time – see check list above.  If you would like to enroll or cancel please contact Human Resources for an application form. 

2013 Health Insurance: There are NO changes to the health (medical) insurance plans for 2013. There is however a vision hardware administrator change to National Vision Administrator, NVA. The City did find through a request for proposals that we are able to save approximately $20,424/year by using NVA for vision hardware (vision exams are covered by medical plan not the hardware plan). Click here for the summary of the existing hardware plan versus NVA's plan. You will see that they were able to match the benefit design exactly while at the same time adding new benefits. Additional benefits (on page 1) include hardware (lenses) covered once every 12 months vs. 24 months - regardless of RX change or not. Additional benefits (on 3rd page) include fixed fees for special lens options (when in network), optional mail order contact lenses and fixed costs for hardware above and beyond what the plan covers (for example if a person wanted a second pair of hardware in one year - without a prescription change).

Vision Providers: SHP's plan has five providers two of which are Marshfield Clinic owned (Marshfield Clinic Optical and Marshfield Eye Center). NVA has 59 pages of providers (click here) nationwide that employees could seek hardware at - with four providers in Marshfield. In addition, there are providers in Pittsville, Stevens Point and Wausau etc. Please note that NVA does not list Marshfield Clinic owned optical centers for hardware due to their pricing. Just to be clear - an employee can still have their vision exam at a Marshfield Clinic owned optical center because exams are part of medical - but they would have to go to an NVA provider for the best hardware coverage. If an employee still wanted to go to a Marshfield Clinic owned center for hardware, they could do so but it would be covered as out of network.

Annual Notice of Creditable Coverage Disclosure: (Medicare Prescription Drug Coverage). For those covered on the City’s health insurance plan and also covered under Medicare.  (Click here for full disclosure notice.)

2013 Dental Insurance:  Dental insurance premiums will not increase in 2013 however there is a plan design change specific to orthodontia. In the past the orthodontia benefit read "Delta PPO Dentist = $695 deductible then 100%; Delta Premier Dentist and Non-network Dentist = 50% to $1,500 Max." The new benefit will read as "Delta PPO Dentist = 50% to $3,000 Max; Delta Premier Dentist and Non-network Dentist = 50% to $2,000 Max." Delta Dental has agreed to grandfather in those who currently have bands in-place. The carrier (Delta Dental) will remain the same.

Wastewater Unit, Ordinance Enforcement Officers Unit, Police Unit, Street, Custodial and Parks and Recreation Group and Fire Unit.

Flexible Benefit Plan:  All Must Complete Form whether participating or not.

FLEX Administration: The FLEX administrator will continue to be Employee Benefits Corporation (EBC). WI state law requires employers insurance plans to cover children (their child only not their child's spouse or their grandchild) through the end of the month they attain the age of 26 and therefore employees can plan for FLEX dollars for their dependents up to age 26 (even if they don't qualify as dependents for IRS purposes). If a child turns age 26 in plan year 2012, they are not eligible for FLEX reimbursement starting on January 1, 2013. Group Insurance (health and dental) Premiums: Group insurance premiums will be deducted pre-tax automatically. Email or contact Lara Baehr at 486-2004 if you do not want your group insurance premiums deducted pre-tax.

Medical Expense Flexible Spending Account: You MUST re-enroll in the program each year to participate. The annual maximum is $2,500 for 2013 due to mandatory federal health care reform.

Dependent Care Plan:  You MUST re-enroll in the program each year to participate.  The annual maximum is $5,000.
